It's clearly a poison lozenge. Much like the better-known poison rings, this lozenge has a built-in compartment in which to store deadly poisons. A would-be assassin need only conceal the lozenge in his/her throat and set about the task of finding the individual in need of poisoning.
These lozenges were created by the Coco Ninja Clan as a way of making their assaassins less likely to waste company time chatting with geishas & whatnot when they should be killing folks. Within weeks of the implementation of this product, it became very clear to clan leaders who the slow pokes were (or had been, as the case may be).
